We have a detailed intake process that is designed to improve efficiency and provide the best service possible. In order to set up an appointment and receive an appropriate evaluation, we ask that you create an account on our portal and carefully fill out all of the forms as completely as possible. Once you complete the forms you will click save and submit. Once we receive your intake, our team will review the information for the appropriate clinician while the billing department reviews your covered benefits. You will be contacted by one of our team members as soon as an appointment date becomes available.
